Online Loan Agreement

I hereby consent, in the event of applying for a loan  (including for an overdraft, line of credit, or credit limit):

• For the EVOCABANK Open Joint-Stock Company (hereinafter referred to as the Bank) to inquire  and obtain information from information systems as soon as this consent is granted until a contract is concluded with the Bank (for the purpose of reviewing the application for a loan, credit, and/or other financing), as well as in the event of concluding a contract with the Bank — throughout its entire duration of the loan agreement. These inquiries pertain to my current and past financial liabilities, as well as other data that may be taken into account by the Bank when making a decision to enter into a loan agreement (or other contracts) with me, as well as for developing proposals to enter into relationships that entail monetary obligations;
• For the Bank to receive without my prior notice information on my future liabilities and other data from information systems during the effectiveness of the loan agreement (or other contracts). I am well aware that the provision of information and data depending on their contents can affect a relevant decision of the Bank;
• For the Bank to obtain from the Nork Information and Analytical Center CJSC and Ekeng (Electronic Governance Infrastructure Implementation Agency) CJSC any information regarding my personal data, including passport details, public services number (social card number), viability status (alive/deceased), date of birth, date of death, gender, citizenship, code of place of residence in the RA, province of my residence in the RA, settlement, street, house, apartment, employment and marital status data, information on paid income tax and cumulative pension contributions, number of hours worked, salary and equal payments, information on payments under civil-law contracts, photograph on an identity document, and information on my income; and I agree to their processing by the Bank, including their collection, input, systematization, storage, transformation, restoration, transfer, destruction, and use for the purposes of identification, decision-making, opinion formation, with the aim of acquiring rights, granting rights or benefits, restricting or depriving of rights, etc. At the same time, the Bank has the right to collect personal information by any lawful means, whether by receiving it directly from me or from other sources not prohibited by law, for its use and review in any process related to the provision of services, and this shall not be considered a violation of the RA Law 'On Bank Secrecy' or the RA Law 'On Personal Data Protection'."

I am aware of and accept that my personal data may be transferred by the Bank to ACRA Credit Reporting CJSC and the RA Central Bank. This consent/personal data may be revoked, rectified, destroyed, archived, and data processing may be terminated after the cessation of any legal relations with the Bank based on my written application, in accordance with the procedure of the Bank and the RA legislation.

I hereby confirm that I am familiar with the essence and importance of credit history and credit scoring, the type of scoring applied, and the factors affecting the score. I am also aware that inquiries to ACRA Credit Reporting CJSC, as specified in the table below, may affect my credit score.

I hereby consent to receive updates on my loan, as well as information on products and services offered by the Bank, via email and/or SMS messages to my phone number.

The data operator is EVOCABANK OJSC (address: 44/2 Hanrapetutyan street, Yerevan, RA). I have read this consent form, fully understand its terms, and agree to them.
